  • 2º Mobitz type II atrioventricular block (AVB) is a pathological arrhythmia, typically caused by damage to the His-Purkinje conduction system
  • Characterised by intermittent non-conducted P waves resulting in sudden dropped QRS complexes
    • In comparison to Mobitz type I where there are progressively prolonged PR intervals before the dropped beat
  • It is considered pathological so investigating the underlying cause is essential
    • Common causes: myocardial fibrosis, inflammation, drugs, neoplasia
  • Clinical signs including lethargy, exercise intolerance and syncope are due to bradycardia and reduced cardiac output
  • Diagnosis is based on characteristic ECG findings
  • A permanent pacemaker is the only definitive long-term treatment
  • There is a risk of progression to 3º AVB and sudden cardiac arrest
